COMPOSITION:  Each soft gelatin capsule contains:
Alfacalcidol B.P. 0.25 mcg  (1µ-Hydroxy Vitamin D3)
Calcium Carbonate I.P. 500 mg  (from Oystershell) equivalent to elemental calcium    200 mg

 

Calcium Carbonate (CaCO3) is used as a calcium supplement, as an adjunct in the treatment of osteoporosis.
Calcium is needed for the mineralisation of bone:

  • Increases calcium absorption from the G.I.T.
  • Monitors calcium deposition on the bones.
  • Reabsorption of calcium in the kidneys.

Metabolism of Vitamin D

Vitamin D can be obtained from the dietary sources such as eggs, butter, cheese, fish liver oils. Moreover, it is also synthesized in the human body in the form of Vitamin D3 which is metabolized to the active form 1µ, 25- dihydroxy cholecaliferol  first in liver and then in kidneys.

Alfacalcidol is 1µ-Hydroxycholecalciferol i.e. only hydroxylation in liver is needed to convert alfacalcidol to active form of Vit. D
3.1µ 25- dihydroxy cholecaliferol in intestines   increases calcium binding in proteins thus increases absorption of calcium and phosphate.In kidneys its increases reabsorption of calcium and thus increases deposition of calcium in bones.CaCO3 fulfils additional calcium demand of the body.

Alfacalcidol is fat soluble Vitamin, extensively absorbed due to its high solubility, it is rapidly distributed throughout the body.Radiolabled studies indicate that within 30 minutes it starts getting converted into calcitriol and reaches peak at about 12 hours.13% of Alfacalcidol converted to calcitriol is excreted in urine and 4% in faeces in the form of metabolites.

Conversion of Alfacalcidol to calcitriol is not significantly affected in case of hepatic insufficiency.Calcium Carbonate is converted to calcium chloride by gastric acid. Some of the calcium is absorbed from the intestine and rest is reconverted to insoluble calcium salts and is excreted in faeces.Calcium has bioavailability of about 30%. Normal levels of calcium in the blood is between 9-11 mg / 100 ml of blood. It is eliminated quickly through renal excretion.
